The Interstate Medical Licensure Compact

As of 2026, most states participate, which has meaningfully reduced friction for physicians serving patients across state lines. Compact membership doesn't eliminate the requirement for a license in the patient's state — it makes obtaining that license faster.

The Nurse Licensure Compact

Nurse practitioners and other advanced practice nurses have similar compact arrangements through the Enhanced Nurse Licensure Compact. Compact membership allows NPs to practice across state lines under a single multistate license. Coverage is broad but not universal.

States That Remain Restrictive

A minority of states have chosen not to join major licensure compacts or maintain additional requirements beyond compact provisions. Patients residing in these states may find telehealth options more limited to providers who have taken the additional step of obtaining state-specific licenses. This particularly affects niche or specialty telehealth services where the addressable market in one state may not justify additional licensure costs.

The Prescription Angle

Prescription authority follows licensure. Prescribers must be licensed in the state where the patient is located at the time of the visit to write valid prescriptions.
