The distinction most people miss

When someone says "I need a therapist," what they often mean is "I need medication management for anxiety/depression." These are completely different services provided by completely different professionals — and most telehealth "mental health" platforms only offer one.

Therapy = talk therapy with a licensed therapist (LCSW, LPC, LMFT, psychologist). Cannot prescribe medication. Psychiatry = medication management with a psychiatrist (MD/DO) or psychiatric nurse practitioner (PMHNP). Can prescribe. Most patients with moderate-to-severe depression, anxiety, ADHD, or bipolar disorder need both — but telehealth platforms rarely make this clear before you sign up.

The access gap: There are approximately 30,000 practicing psychiatrists in the US serving 50+ million adults with mental illness. Wait times for a new psychiatry appointment average 6-8 weeks in urban areas and 3+ months in rural areas. Telehealth can help — but only if the platform actually employs psychiatrists.

What to look for

Before signing up for any telehealth mental health platform, verify: Does the platform employ board-certified psychiatrists or PMHNPs? What's the actual wait time for an initial appointment? Is ongoing medication management included, or only the initial prescription? What happens if your condition worsens — is there an escalation pathway?
